Department of Mental Health Adult Services (Ages 19+) DMH Adult Services focus on community-based supports which enable individuals receiving treatment to recover and thrive in their communities.
Adult Full Service Partnership (FSP) programs are designed for adults ages 26-59 who have been diagnosed with a severe mental illness and would benefit from an intensive service program. The foundation of Full Service Partnerships is doing “whatever it takes” to help individuals on their path to recovery and wellness.
DMH Application Forms Request for Services Forms are used when applying for mental health services from the Department of Mental Health (DMH) for adults, children, and adolescents. Instructions for submitting an application are included with each application.
